Lead generation for Medicare agents and FMOs
Medicare lead generation is high-volume, AEP-seasonal, and heavily CMS-regulated. Here's how it actually works in 2026.
The Medicare lead market in 2026
Medicare lead generation is one of the largest sub-sectors of U.S. consumer lead gen, driven by ~65M Medicare beneficiaries plus ~10,000 new enrollees per day aging into eligibility. The buyer pool is large but heavily regulated by CMS.
Who buys Medicare leads
- FMOs (Field Marketing Organizations): Integrity, AmeriLife, Senior Market Sales, TogetherHealth.
- Direct-to-consumer brokerages: eHealth, GoHealth, SelectQuote.
- Independent multi-carrier agencies. Owner-operator decides. Volume: 10-50 leads/mo.
- Captive agent networks (UHC, Humana captive teams). Regional sales director decides.
Pricing as of 2026
- Shared form leads: $15-$40.
- Exclusive form leads: $35-$80.
- Phone-qualified leads: $40-$120.
- Live transfers (licensed agent ready for SOA): $80-$300.
- AEP (Annual Enrollment Period, Oct 15 - Dec 7) pricing typically 30-100% higher than non-AEP.
What makes a qualified Medicare lead
- Age 65+ OR qualifying for Medicare (disability, ESRD).
- Within an applicable election period (AEP, IEP, SEP, OEP).
- Specific ZIP matches buyer's licensed-state coverage area.
- Has prescription drug list available (for Part D conversations).
Channels that work
Google Search (heavy AEP seasonality), direct mail to turning-65 lists, linear TV and CTV (senior demographic still consumes broadcast heavily), podcast advertising on senior shows, AI Search Optimization (emerging — seniors are slower adopters but the market is growing). Avoid most TikTok/Reels — wrong demographic at scale.
Compliance gotchas
CMS rules are extensive. SOA (Scope of Appointment) must be completed before discussing any plan. PTC (Permission to Contact) must be on file before outbound calls about MA/Part D. Recording compliance: most Medicare sales calls are recorded and stored for 10 years per CMS. Don't imply official Medicare affiliation. Recent CMS final rules (2024-2025) tightened TPMO (third-party marketing organization) disclosure requirements.
